What will a Care Manager do for me or my loved one?
Upon completing a comprehensive psycho-social assessment, a Care Manager will work to develop a Care Plan to meet the needs of each individual client and family, which may include:
- Coordinating In-Home Services
- Accompanying/Transporting to Medical Appointments
- Client Advocacy
- Discharge Planning After a Hospital Stay
- Long-Term Planning
- Ongoing Monitoring and Support
What Makes ElderCare360® Different from Other Programs?
We are accredited by the Council on Aging (C.O.A.) and have been serving the community and providing care management services since 1974. Our team is comprised of skilled professionals who have a wealth of experience and expertise working with older adults and their families. We believe that the Care Management relationship develops between the professional, older adult and their families, forming a triangle of communication, support, and guidance.
What are your fees?
Our fees for Care Management are billable per hour and are pro-rated in five-minute increments. Much of a Care Manager’s time is not only spent on face-to-face visits, but also on phone calls necessary to help coordinate services. We take time up-front to work with our clients and their families to discuss and tailor the frequency of the Care Manager’s visits/calls based on individual wants and needs. An itemized bill outlining all charges is presented the first week of each month.
